Historic coverage expansion
Starting July 1, 2026, Medicare launched the GLP-1 Bridge program, a time-limited demonstration that provides eligible Part D beneficiaries with access to certain GLP-1 weight loss medications for approximately $50 per month. Previously, these drugs could cost over $1,000 monthly out of pocket. The program runs through December 31, 2027, and covers medications including Wegovy and Zepbound.
Eligibility and enrollment
To qualify, beneficiaries must be enrolled in Medicare Part D and meet specific weight and health criteria. Eligible individuals must have a body mass index meeting obesity thresholds or weight-related health conditions. The program requires a doctor's evaluation and prior authorization. Importantly, Part D plans do not need to opt in — eligible beneficiaries can access these medications directly through the demonstration program.
Impact on public health
The Medicare GLP-1 Bridge addresses a long-standing gap in coverage, as Medicare has traditionally not covered medications solely for weight loss. With obesity affecting more than 40% of American adults, the program could have far-reaching public health implications. CMS officials say the pilot will generate data on outcomes and costs, potentially informing permanent coverage decisions. However, some FDA scientists have raised concerns about unregulated peptide alternatives entering the market through parallel channels.
