New research suggests that menopause-related changes in the gut begin years before a woman's final menstrual period. In a study of 964 women followed for nearly 25 years, researchers tracked two blood markers linked to gut barrier dysfunction and exposure to microbial products. Both markers were stable for years, then began rising in the period leading up to menopause and stayed high afterward. The work adds to a growing picture of menopause as a gradual, whole-body process that begins well before its usual outward signs appear.
Two blood markers shift early
The marker FABP2, which signals damage to the intestinal lining, began rising about 2.5 years before the final menstrual period. A second marker, sCD14, which reflects the movement of bacterial products out of the gut, started rising about 2 years before that milestone. The timing points to a long transition rather than a sudden change at the moment periods stop. The study, titled 'Markers of compromised gut epithelial barrier integrity increase during the menopause transition,' follows women from their reproductive years through the period after their final menstrual period.
Levels stay high after menopause
About six to six and a half years after the final period, both markers leveled off at readings higher than those seen before menopause. The rate of change was not linked to race or ethnicity, the age at which menopause began, or body mass index, suggesting the pattern is common across women.
What the findings mean
The authors examined whether a weakened gut barrier could allow more microbial material into the bloodstream. They found a clear link: for every 10 percent increase in FABP2, sCD14 rose by 2.6 percent. That pathway matters because chronic low-grade inflammation is tied to many conditions that become more common in midlife, including heart disease and metabolic problems. The researchers described the menopause transition as a period of rising gut barrier dysfunction and said the findings could guide future work on diet, gut health, and inflammation in women approaching menopause.
